12 Deadlifts 135/95 *Health = Dumbbell *Moderate Load
9 Hang Power Clean 135/95 = *Health = Dumbbell *Moderate Load
6 Push Jerks 135/95 = *Health = Dumbbell *Moderate Load

Two way to do this..

Partner 1 can do 1 full round then partner 2 can do a full round or Partner 1 can do all 5 rounds then partner 2 does all 5 rounds.
